Many organizations conduct exit interviews; but at that point, it's too late. People want to give feedback, but not when it doesn't matter anymore. So rather than asking why people are leaving, there’s a more important question to ask: Why do good team members stay?

Getting regular feedback shows your team that you care about what they have to say. It's important to get feedback that will provide useful information that can help you keep your finger on the pulse of your culture, measure progress of culture initiatives, and give you direction to re-energize your culture.
